Here’s a question I get asked all the time: “Why do I get dark circles under my eyes?” Well, it’s not about being tired… On peterattiamd’s podcast (The Drive # 355) with suzanobagimd, we explained the 4 main causes of infraorbital (under-eye) dark circles: 1️⃣ Visibility of the orbicularis muscle 2️⃣ Shadowing 3️⃣ Hemosiderin (iron) deposition 4️⃣ Hyperpigmentation Years ago, I published a review article on this topic with a couple of our toceyeandface fellows that has been cited over 100 times (you can download it for free on Google Scholar/PubMed and in my Linktree). Combined with ptosis (droopy eyelid) and skin aging changes, it was time for a tailored solution. I’m a believer! 🤩 Skincare is the first line of defense against aging. As a follow up to my recent podcast with peterattiamd — let me share the core skincare routine I suggest for my patients and how to layer products: Swipe 👉🏽 to see the typical order I recommend for product application! AM ROUTINE ☀️ 🔹 Cleanser (gentle) 🔹 Serum (Vit C + Peptides) 🔹 Mineral-based sunscreen (UV protection is VERY important. Why mineral? Listen to the podcast!😊) PM ROUTINE 🌙 🔹 Cleanser (gentle again) 🔹 Serum (Vit C + Peptides) 🔹 Retinoid or prescription treatment (OTC is fine for starters) 🔹 Moisturizer I generally don’t recommend aggressive cleansers because they can overly strip oils and can damage the skin microbiome. After washing, then layer on the thinner 💧water-based products, and finish with thicker moisturizers that lock down the active ingredients below. What brand to use? I feature my own line in my profile, but honestly I support any thoughtfully formulated 🔬science-forward line, and our podcast show notes list many options. 🧴✨ Spend your extra dollars on serums which are complicated to formulate (but carry extraordinary benefit.) ✔️ Skincare routines don’t have to be complex. 😃Stick to these core steps and you’ll be giving your skin the foundation it needs to stay healthy, radiant, and youthful. Oculofacial plastics is a creative discipline, and we love innovating new techniques!💡Last Friday many toceyeandface surgeons travelled to the ASOPRS 2025 Spring Meeting in Napa Valley. My daddy duties took priority this past weekend but our senior fellow makaylamc.md presented three cutting-edge surgical techniques that my team and I developed this past year! 1️⃣ Electrocautery for Prominent Eyelid & Facial Veins: Visible blue veins around the eyes 👀 and cheeks can be frustrating and tough to treat safely and effectively. Rich Allen (oculosurg) came up with a unique approach that uses an IV catheter and electrocautery ⚡️ to precisely treat these veins without surgery or scarring. We utilized his technique this past year and it’s safe, simple, and effective: no lasers or incisions needed! (👉🏼Swipe for images) 2️⃣ Custom Orbital Onlay Implants for Thyroid Eye Disease (TED): In the most severe cases of TED, routine decompression surgery isn’t enough. We worked with strykercmf to develop a multi-piece custom-fit implant that sits along the orbital rim 💀 to further reduce eye protrusion. The result is a vision and life-changing surgery! 3️⃣ Modified Weber-Ferguson Incision for Fibrous Dysplasia: Fibrous dysplasia 🦴 is a rare bone condition that can distort the face and affect vision and breathing. Anish Abrol (dr.anishabrol) and I designed a very unique surgical approach to remove excess bone in the cheek and orbital area. This technique offered incredible surgical exposure 🌟 while minimizing risks to the facial nerve. A couple of years ago, I had the massive privilege of serving as the inaugural speaker for the Norman Shorr Lectureship at the UCLA Stein Eye Institute. 🎤👁️‍🗨️ UCLA is where I completed my residency & fellowship nearly 20 years ago 🩺👨🏽‍⚕️—and I trained under two giants of oculofacial plastic surgery, Dr. Bob Goldberg and Dr. Norman Shorr, whose mentorship shaped my entire career. (They are known as Bob & Norm to those who know them dearly) In my lecture, I reflected on the enduring impact of Dr. Shorr’s teachings - I literally think of him daily when I talk to patients and perform surgery. Medicine is a generous discipline - we all share knowledge freely. My mentors shared with me, I share with my fellows, and the cycle goes on and on! Some key teachings from Norm: ⭐️ First seek to define the problem, then solve it. ⭐️ Get ten years of experience in your first five years. ⭐️ Operate obsessively for the very best outcome. ⭐️ Do not fear a patient’s psychological trauma from prior failed surgeries (Norm specialized in fixing “botched” eyelids) - instead, earn their confidence via sincere empathy, and help them understand and accept their anatomy, then aligning on attainable goals. WHAT IS THE SCIENCE OF HUMAN ATTRACTION? 🧠✨ Our attraction to beauty isn’t just cultural—it’s hard-wired at birth. 👶🏼👁️ Evolution has shaped our preferences over millennia, and science tells us we’re drawn to specific facial features for very primal reasons. As animals, we tend to find three features attractive in potential mates: 🔹 Symmetry – Faces that are more symmetrical may signal good health and strong genetics. 🧬 Studies show we consistently rate symmetric faces as more attractive. 🔹 Averageness – This means a blend of diverse traits. Think of composite faces made from combining many individuals—these faces are often perceived as the most attractive. Why? Because genetic diversity may signal resistance to disease and better long-term health. 🦠🛡️ 🔹 Gender Traits – Masculine features like strong jaws in men 🧔‍♂️ and feminine features like full lips and large eyes in women 👩‍🦰 are often seen as attractive because they signal fertility, strength, and reproductive potential. Much of this was outlined in a fascinating series of scientific studies in the 1990s, especially by researchers like Thornhill and Gangestad. 📚🔍 As a oculofacial plastic surgeon, I enthusiastically connect the art of facial aesthetics with the science of socio-biology. Just had an absolute blast traveling to Istanbul this past week for the 2nd ever World Society of Ophthalmic Plastic Reconstructive and Aesthetic Surgery (WSOPRAS)!
