5 tips to prevent diaper rash #DYPERpartner

1 - Pick the right diaper - Choose a fragrance free, lotion free, hypoallergenic diaper that is super absorbent. Ideally, one that is mostly plant based materials without a plastic or dyed inner liner.  My top recommendation for an effective and gentle diaper that meets these high standards is the get.dyper brand. 

2 - Change your baby’s diaper frequently - every time it’s wet or every few hours.

3- Apply a bland barrier ointment with every diaper change, something gentle and thick - like plain petroleum jelly or a zinc oxide paste.

4-Avoid using products with fragrance or essential oils in the diaper region - for example, stick to baby wipes that are fragrance free.

5- Bathe your baby regularly, daily to every other day, this helps remove dirt, debris and microbes, be sure to help strengthen the skin barrier after bathing with a thick barrier ointment

When to Worry? If your baby has a diaper rash that is not going away, is very red, spreading or has sore or blisters, it’s time to see your doctor. Thankfully, that is not often the case and most babies do really well following the techniques described above!

Last week, the unimaginable happened in my neighborhood. Two children were murdered, 21 children and churchgoers seriously injured while assembled in prayer at their first all-school mass. 

Annunciation Catholic School is part of the Minneapolis community, many children on my street attend school and worship there. As a mom of two school aged boys and a physician, I held my breath and still feel as though I cannot exhale. 

The community is grieving, there is so much pain. There is a feeling in the pit of our stomachs and in our moral compasses about how wrong this is. 

It is wrong that our lawmakers repeatedly allow this to happen. 

The number one cause of death for children in America is gun violence, and it is something that the American people have the power to change.

We must love our children more than guns. 
We must act. 
We must demand change. 
We must vote for those who will protect our kids - it should be our #1 priority.

Yesterday I was giving my annual resident lecture on treating severe atopic dermatitis, I remembered I had included this note of gratitude in my presentation. 

It was given to me by a child I treated several years ago with intensive topical therapies: Daily bathing, diluted bleach baths, topical anti- inflammatory medications and wet wraps. 

She did great- and she drew this card for me showing how much she felt her skin had improved.

This little note remains one of my most prized possessions; it's the type of compliment that I live for! 

These are the small things that keep me going and help me find purpose each day.

What's a skincare smoothie?

It's a viral skin care trend, where influencers - and even skin care companies - propose that you use multiple products and apply them all at once haphazardly on the skin. 

The problem here is that most skin care formulas are not tested to be applied in this manner. Most serums or creams are meant to be applied in a certain order, and applying numerous active ingredients may inactivate them, lead them to become more irritating, or make them lose efficacy altogether. 

This seems like a waste of money and time. 

One of the more concerning aspects of this trend is that this type of behavior seems to be heavily marketed to tweens and teens. ...

This post is for all the new moms out there!

As a pediatric dermatologist, I know the newborn period is a time when Moms and caregivers have a lot of questions about how to best care for their baby's skin. 

Choosing a diaper cream is really important. 

Think about it.: this is a substance that you will be putting on your baby's skin, sometimes when it's open and eroded during an active rash, multiple times a day, with every diaper change. And then, after securing the diaper, the diaper cream is occluded against your baby skin. This makes the different ingredients in that diaper cream even more potent. If you've been following my page for a while you'll know the three things that I  recommend looking out for.

1-avoid fragrance
2-avoid essential oils
3-avoid irritating preservatives

Excluding these three types of ingredients will really help when it comes to choosing a safe and effective diaper cream!

What would I tell you as a pediatric dermatologist if I wasn't afraid to hurt your feelings? 

Here are a few pearls...

1-Milk baths for eczema are not very helpful. There is much better data on the use of diluted bleach baths for patients with atopic dermatitis and skin infections. I recommend dilute bleach baths every single day. 

2-I've spent a lot of time on my SM trying to help educate parents that fragrance is not beneficial in any way to their baby's skin and it might be causing more harm than good. Allergic and irritant contact dermatitis and potential for absorption are three reasons why you should avoid using fragrance on your baby's skin

3-This one is pretty obvious. The so-called Sephora kids trend where gen alpha tweens and teens are overusing harsh ingredients is still going strong. Tween skin does not need harsh, exfoliating acids, toners, scrubs, peptides, retinol etc! 

4-Finally, there is so much misinformation circulating online about topical steroids. The fact is that topical steroids can be used safely and so effectively as a first line therapy, in almost all inflammatory skin condition, including on children and babies.

Baby wipes!

So many good options out there. 

My recommendations when reading labels?

Prioritize:

✅Fragrance free
✅Essential oil free
✅No surfactant (ie: SLS or cocobetaine)
✅Hypoallergenic 

Avoid wipes that have numerous botanical extracts too. All of these extra ingredients are unnecessary and increase the risk of skin reactions!

The Pink Vaseline marketed specifically for babies has synthetic fragrance. This highlights the growing concern that claims on personal care products- even babycare products- hold very little to no meaning. 

What does it mean when they put 'pediatrician recommended' right on the label? unfortunately, it doesn't mean much. These claims are not standardized or regulated in any manner so they truly don't hold any weight.

The plain original Vaseline petroleum jelly is an excellent and very safe all purpose ointment. From chapped lips to moisturizer to diaper barrier to wound care to slugging- it literally is a top skincare multitasker.  It's definitely dermatologist AND pediatrician recommended!

As a board certified pediatric dermatologist here are five simple things I wish all parents of babies and children with eczema understood 

1-Food allergies do not cause eczema
Despite so much confusion, the opposite is actually true; an altered skin barrier in the setting of eczema/atopic dermatitis is what predisposes children to developing food allergies 

2-essential oils should not be applied to baby skin. Essential oils are concentrated botanical extract that can lead to irritation or allergic contact dermatitis when applied directly to the skin. They also may be absorbed and have biologic activity that has not been studied in babies. 

3-Daily bathing is helpful for children with eczema. This one has been studied and frequency of bathing matters! A daily bath plus moisturizer is a very effective way to soothe a dry skin barrier

4-Vaseline is the safest and most effective moisturizer for baby skin. Vaseline is triple refined, and extremely safe. It is free of contaminants. It provides a thick occlusive layer on the skin that prevents transepidermal water loss and allows the skin barrier to heal underneath. 

5- Cradle cap may be an early sign of eczema/atopic dermatitis. This is something I see frequently in my clinical practice. Early onset, and/or severe cradle cap may be the first sign of skin barrier dysfunction, predicting the development of eczema. If your child has severe cradle cap, see your doctor
