Skeletal muscle mass accounted for 45.6% of my body weight last month: 20.2kg out of 44.3kg. A month later, the figure had risen to 20.5kg, while total body weight had reduced slightly to 44.0kg. This shifted the ratio to 46.6%. I changed nothing during that time except for one variable: Pure Impact sessions since bringing the device into clinic.

I am not presenting this as scientific evidence. It is a single data point, and I take it at face value (although I will continue to collect data points as I enter maintenance phase with Pure Impact). Those who regularly track their body composition will know that increasing muscle mass, even marginally, without gaining weight and without altering diet, alcohol intake, or training is difficult. Particularly in women in their forties. For me, this small shift reflected what I was beginning to notice anyway: better muscular recruitment, faster recovery, improved physical resilience, and surprisingly, sleep.

This is not about weight loss or appearance, but about function. Skeletal muscle is the largest organ in the body by mass, and its role extends far beyond aesthetics. It is fundamental to metabolic health, regulating more than 75% of insulin-mediated glucose disposal. It plays a central role in mitochondrial function, immune response, and inflammation modulation. Low muscle mass is now recognised as an independent predictor of poor outcomes across cancer care, critical illness, cardiovascular disease, and even surgical recovery.

Despite this, muscle is rarely measured, and almost never discussed in routine clinical care. We track weight, BMI, blood pressure, cholesterol, but not structure. Most adults begin losing muscle mass in their thirties. By the time its absence becomes clinically relevant, the decline is often advanced. I share this not because I believe a single device or treatment is the answer, but because I think the conversation needs to change. Muscle is not just a fitness goal and it’s most certainly not an aesthetic one. I am not an athlete. I have never been on a sports team, and I have no desire to run a marathon or sign up for Hyrox. I train for my health. 

At 45, I am more certain than ever that strength matters. I do not train to look smaller. I never have. What matters is what my body can do, how well it performs and recovers, and whether it will carry me through the next decades with stability and confidence.

Muscle is not about aesthetics. It is about health. It improves insulin sensitivity, supports bone density, and protects against the decline that comes with age. It is one of the most reliable ways we have of maintaining independence and resilience over time.

I am not training for a competition or to perform online. I am a doctor who lifts weights because it helps me stay strong, focused, and functional. I train consistently, and I expect results to come from effort over time. When I invest in new clinic technology, it is because I intend to use it myself. I spend tens of thousands of pounds of my own money on devices that meet the standards I hold for long-term health, structure, and performance. If I would not use it on my own body, I would not offer it to patients. Self London has always been about the whole self.

Pure Impact is the latest addition. It uses high-intensity electrical muscle stimulation to contract and recruit muscle fibres more deeply and intensely than most people can achieve voluntarily. This is not a fat loss tool. It is not about shrinking. It is about building muscle, improving strength, and sculpting the body through better definition, activation, and control.

I am using it alongside my regular strength training. My baseline data, including skeletal muscle mass, body composition, and gym performance metrics, have been recorded. I expect to lift heavier. I expect my muscle-to-body weight ratio to improve. I am looking forward to seeing what it adds to both recovery and output.

I’m delighted to announce the newest energy-based device to self.london - Pure Impact by Sofwave.

I did not know until it happened to me.

1. 14x as many women die from heart disease as from breast  cancer.
2. Elevated lipoprotein a is a silent risk factor which affects 1 in 5 people 
3. Women often present differently, leading to misdiagnosis and dismissal.
4. The data we rely on to guide treatment is largely based on male patients.

Six weeks into investing my savings into the clinic and opening self.london I needed a coronary stent. I’m delighted today to have had the opportunity to speak as a patient advocate to the World Heart Federation. 

I don’t think awareness for women is where it needs to be and I don’t think for many people with an early family history of coronary artery disease (regardless of gender), we are screening early enough.

Know your risks, ask questions, and push if something feels off. Coronary artery disease develops silently (until it’s a problem and starts to cause symptoms such as difficulty breathing or chest pain or discomfort), keep an eye on your lipids and blood pressure as a bare minimum and think about where in your lifestyle even small changes can be made.

I’ve been preparing a talk for the World Heart Federation today to deliver later this week as a patient, and it has unexpectedly made me pause. It is just over two years since I had an unexpected coronary angioplasty and stent. I was 43. self.london had just opened. I had no major risk factors. But something did not feel quite right, so I listened. 

And I am glad I did.

Since then, life has continued: patients, responsibilities, clinic life. But my relationship with time, energy, and decision-making has changed. Living with a long-term health issue while running a business teaches you what is worth carrying and what is not.

If I could go back and speak to myself in the early days of that diagnosis, this is probably what I would say:

1️⃣ Life is short. Not in a dramatic way, but in a very real one. Use your time well.
2️⃣ No one else is going to take care of your health, your boundaries, or your goals. You have to lead on that and be responsible and accountable.
3️⃣ Entitlement is not a strategy. The world does not owe you anything, even if you do everything right.
4️⃣ You can make the best decision available to you and still not get the outcome you hoped for. Outcomes are never guaranteed, but that does not mean the decision was wrong.
5️⃣ You can be compassionate and still have limits. Saying no is sometimes the most responsible thing you can do.
6️⃣ Building something that reflects your values takes work. There are no shortcuts if you want to do it properly and remain true to your core values.
7️⃣ Confidence in women, especially in business, is often misread; directness becomes arrogance, clarity becomes coldness. But you do not need to perform softness to be taken seriously.
8️⃣ Pay attention to what people do, not what they say. Actions always consistently speak louder than words. “Do-ers” over “say-ers” every time.
9️⃣ Shared values matter. Whether in friendships, work, or hiring, they shape everything.
🔟 Nothing is guaranteed. Not health, not certainty, not even tomorrow. But you do not need guarantees to live well.

2024 has been one of the most demanding yet fulfilling chapters of my career. Creating Self London has been a deeply personal journey. Every detail, from the flooring to the blinds to the taps, reflects my vision, but it’s the heart of the clinic—our team and the care we provide—that fills me with the most pride.

What drives me isn’t external validation but a quiet, inner belief in showing up and doing my best each day. Life has its ups and downs, and I’ve learned to find peace in that rhythm. I remind myself often that I am just a tiny part of something much larger. This perspective keeps me grounded, diminishes any false sense of self-importance, and fosters a deep sense of gratitude for all that I have.

Turning 45 in a few days feels like a milestone, not because I have everything figured out, but because I’ve come to embrace resilience as a way of life. Each decade has shaped me in ways I couldn’t have predicted. Losing a parent as a child, enduring bullying in my teens, navigating the intense years of training as a junior doctor, calling off a wedding, failed IVF, and facing heart disease — every struggle has taught me something invaluable.

These challenges have shown me the strength in vulnerability and the power of perseverance. My own skin struggles gave me the empathy to connect with my patients. Long hours and hard work taught me there’s no substitute for experience. Knowing what I didn’t want has guided me towards what I truly value. Standing firm in my beliefs, even when others doubted, has been worth every step of the journey.

My life isn’t perfect, nor do I think anyone’s is. But I’ve realised that how we respond to life’s challenges is what matters most. My life is no easier or harder than anyone else; we all have our stories. Resilience isn’t about being unshakeable—it’s about learning to bend, to adapt, and to rise again. My hope is simply to live a life of integrity and leave the world a little better than I found it.

Restart a Heart Day, October 16th

Early 2023, I’d be walking up the hill from Alexandra Park station to the front of the palace and halfway up, I’d feel breathless, tight-chested, and would need to stop—especially when the weather was cold. Fast forward to yesterday: I walked up that same hill without thinking twice, not out of breath, no pain, just grateful I got myself checked when I did.

Turns out, I had severe narrowing of my LAD (that’s a pretty important artery) with evidence of soft plaque rupture, putting me at risk of sudden cardiac death. A sobering thought, but an intervention and stent later here I am. Lifelong meds, medical appointments, and a few side effects. (An endoscopy and iron infusion weren’t entirely on 2024’s road map!).

What I’ve learned is how crucial it is to stay on top of your health. And CPR? That can literally save lives. The British Heart Foundation offers free training, and I reckon everyone should know it. Whether it’s for yourself or someone else, having that knowledge could make all the difference.
So, if you’re feeling a bit off, get checked out. You never know what’s going on beneath the surface. And if you haven’t learned CPR, do it. It’s simple, free, and could save someone’s life.

What an incredible day at the CCR Conference! I’m absolutely delighted to have been part of putting together the Skin Agenda for Day 2, chairing the session, and also speaking about acne—one of the topics I’m most passionate about.

Symptoms of a heart attack include:

Chest pain, pressure or discomfort
Pain which spreads to your left arm, neck or jaw
Feeling sweaty, clammy, nauseous or lightheaded
Breathlessness 

Most heart attacks are caused by coronary artery disease due to narrowing of the arteries. This narrowing occurs due to build up of fatty deposits known as atheroma. If a piece of atheroma breaks off it can result in a blood clot which can block off the artery leading to a heart attack.

Six months ago, we became the first consultant dermatology clinic in Europe to bring in AviClear laser to the clinic. Excited wasn’t even close to how I felt about the capabilities of the technology. My own skin issues aren’t exactly a secret nor is my “why” I do what I do, having lived with acne for over three decades. To be able to have an additional option for acne patients which is drug-free and capable of results like we are seeing is an enormous breakthrough. Fast forward and we are now one of the biggest global users of AviClear with a wealth of data and experience.

I rarely share photos of our patients in clinic here, I hugely value privacy and the journey everyone has with their relationship to their own skin. I am hugely grateful to be able to show you these before and after images just three months after AviClear treatment has been completed (and these just get better as more time goes on; FDA clearance is for long-standing results).

I am proud to have created a clinic where we can offer more than just standard medications (which still have a role to play!) - it is about options - so people can choose what is best for them, but also to then be able to treat the scars in-house with our technologies and offer mental health and nutritional support to our patients.

Twenty years ago to the day - August 4th, 2004 - I started my working career and it was my first day as a doctor in a Welsh valleys town, on a respiratory ward for 6 months. It’s hard to imagine I’ve been working as a medic for this long and yet how that time has passed in the blink of an eye. Medicine is a rollercoaster of a journey, I’d be lying if I said I loved it all - but that’s life isn’t it? You’re not always supposed to enjoy it all the time - lowlights like crashing my car after a week of nights as a medical SHO, feeling unsupported at times in work, and highlights - camaraderie, people (patients and colleagues), learning. This is a great career and I feel like although I had no real “plan” twenty years ago, I’ve ended up exactly where I am meant to be right now, doing exactly what I’m supposed to be doing. Good luck to everyone who is starting out, the ups and and downs will come, but it will be an amazing journey, I promise! I am proud to have supported bsfcharity for coming up to a decade now, in various guises, and will continue to do so in my professional career. This week saw a day out of self.london to talk to school children about the British Skin Foundation’s #moremoremore campaign; more clothing, more sunscreen, more shade and the risks of overexposure to the sun. It was a delight to work with the team, do a double act talking to the kids with lisajbick and have the opportunity to meet HRH Princess Beatrice who is a patron of the charity. Quite a different audience to the one I am used to presenting in front of, but I was taken aback with how much they already knew and how well behaved they were this close to the end of term! Weekend dump - my feed has always been about things that matter to me. In a decade of my online Instagram presence there isn’t a single paid post or a brand telling me what to say here (or worse, being paid to say something but not declaring it). It’s just things that make me who I am - work highlights, health lowlights, what makes me happy and not so happy. ministryofsoundclassical this weekend (having seen this before royalalberthall in the past) made me very happy. This is the first weekend since opening the clinic that I recall doing no work. I didn’t open my laptop, I didn’t write quotes or a blog or a webpage. I didn’t read new research, I didn’t brainstorm what next. I actually let the emails slide, took a break, hung out with my family. If you’re from my generation, you’ll appreciate how good all of this music is - you can’t beat a playlist like this. Whilst I continue to have any type of online presence, this personal page of mine will remain exactly that - personal to me and my values. Those values aren’t going to change. My first real memory of public speaking in a professional capacity was as a first-year med student in 1998. We learned anatomy through human dissection, working in groups of ten (5 on each side of the body). At the end of the year, we had to present our findings and the cause of death of our cadaver. Only one person from each dissection table would be picked to present, and you wouldn’t know if it was you until your name was called. The presentation grade would also be awarded to the rest of your table, so if you screwed up, you would be letting everyone else down too.

I remember thinking 90% odds of NOT being picked seemed good; no one wanted the job of presenting in front of the rest of their year group. But someone had to be that 1 in 10. I still remember my name being called, walking to the lecturn, the bright lights on stage in the otherwise dark lecture theatre, simultaneously feeling like they were burning and blinding me. At 18, I was petrified of looking like an idiot with everyone watching. Imposter syndrome was very real.

Fast forward over 25 years, and I still have a love/hate relationship with public speaking. Inherently, I dislike being looked at, likely a throwback to having had skin issues and feeling scrutinised. Yet, once I’m up and in the middle of it, I find myself peculiarly enjoying it. The cycle repeats every time. I’m still that person up early, practicing in front of the mirror, balancing a laptop precariously in a hotel bathroom to ensure I don’t look foolish on stage at international conferences.

I never realised in the early stages of my career how much of being a doctor is about communication and presentation to groups of all sizes, not just clinical competence. Surprisingly, of all the exams I’ve taken, GCSE drama (an Antigone monologue) turned out to be one of the most useful. It taught me presentation skills, learning lines, and how to hold myself.

My imposter syndrome never goes away, but it drives me to improve. There’s no “perfect” for me in public speaking - just repeat exposure and practice. Why do we use lasers in clinic? There are a huge number of energy-based devices on the market which use light and laser energy for skin rejuvenation, acne scarring, tackling issues of redness or pigmentation. Often one person can have multiple concerns, for example residual acne scars, prominent pore size, fine lines and freckling because of the sun. Combining technologies safely allows for management of both issues of skin tone and texture. Getting good results is all about using the right device in the right person for the right concern. Delighted to have taken part in the Innovations in Dermatology campaign from Health Awareness U.K. I have been knocking around on Instagram for a decade now (one of my first posts in 2014 if you scroll back is about tretinoin!) and over this time I have seen how much misinformation about skin has grown. Our skin never seems to get the same respect as our other organ systems, despite everything it does for us. I won’t take shortcuts with my heart health and I’m not prepared to do it with my skin either. We all deserve to feel good in our skin and not have it limit us in any aspect of our life.
